Why Guardian Health

We help you recover in the comfort of your home

We accept Medicare & Medicaid

Coverage shouldn’t be a barrier to skilled care. We serve both Medicare and Medicaid recipients — including the Medicaid patients many other agencies turn away.

Skilled clinical care

The word health in “home health” is what sets us apart from companion-care and waiver-service agencies. Guardian Health provides skilled nursing — not just aide services.

Serving patients across Indiana

Nurses across all three regions of the state.

Capacity growing as our team grows

30+ years of leadership experience

Backed by decades of home health leadership and clinical know-how at the executive level — the foundation of how we run care.

Shared administrative infrastructure

Guardian Health is backed by the administrative team across all three Guardian brands — powering statewide coordination, billing support, and care continuity.

Community resource hub

Beyond clinical care, we connect patients to Meals on Wheels, home modification services, and other community resources through our sister brands and community partners.

Transparent, physician-led care

Every patient begins with a physician referral and an individualized plan of care — clear documentation, clear expectations, every visit.

Care In Action

What care at home looks like

Skilled, compassionate care delivered where people are most comfortable — at home, surrounded by the people and things they love.

Placeholder photography — replace with your own team and patient photos (with consent) before launch.

What we do

Skilled services, ordered by your physician, delivered at home.

Core clinical services covered for Medicare and Medicaid recipients. Every service starts with a physician’s order and a written care plan.

Wound, catheter & IV care

Skilled clinical procedures in the home as ordered by your physician.

Post-hospital recovery

Care after a hospital stay to help families transition safely back home.

New diagnosis management

Clinical support and teaching after a new or changing diagnosis at home.

Post-operative recovery

Skilled support during the days and weeks following surgery, in your home.

Teaching & training

One-on-one education for patients and caregivers so the right routines stick.

Continuation of care

Working with your medical team to keep care consistent across the care journey.

Home health vs. home care — the difference matters

Guardian Health is a skilled home health agency, not a home care or personal services agency. We do not provide 24-hour care, light housekeeping, respite care, or private duty nursing — those services live with our sister brands, Guardian Care and Indiana Family Caregivers.

Insurance & Eligibility

Will Medicare or Medicaid cover home health?

In many cases, yes. Medicare and Medicaid often cover skilled home health when it’s ordered by your physician and you meet the plan’s criteria. The fastest way to find out what applies to you is to call — we’ll help you understand your options, with no commitment.

Medicare

Covered services as ordered by your physician.

Medicaid

Accepted across the state — including patients many agencies turn away.

Private insurance

Questions & Answers

Frequently asked

Don’t see your question? Call the Guardian Health team — we’re happy to walk through next steps with you.

Does Guardian Health accept Medicare and Medicaid?
Yes. We accept both Medicare and Medicaid — including Medicaid patients many other agencies turn away. Private insurance options are coming soon. Call us and we’ll help you understand your coverage.
How do I get started with home health?
Skilled home health begins with a referral from your physician. You can call us first with questions — we’ll explain the process and what to ask your doctor. Reach us by phone, fax, or text at (317) 669-6685.
What services does Guardian Health provide?
Skilled nursing; wound, catheter, and IV care; post-hospital and post-operative recovery; new diagnosis management; teaching and training; and continuation of care. Every service is ordered by a physician and guided by an individualized care plan.
What’s the difference between home health and home care?
Home health is skilled clinical care — nursing ordered by a physician. Home care (companion care, housekeeping, 24-hour care, private duty) is non-clinical support. Guardian Health provides skilled home health; for the other services, our sister brands Guardian Care and Indiana Family Caregivers can help.
Where in Indiana do you provide care?
We serve patients across Indiana, with nurses in all three regions of the state and capacity growing as our team grows. Call to confirm current availability in your county.
How soon can care begin?
Once we receive a physician’s referral, our clinical team reviews it promptly and coordinates the first visit and care plan. The fastest way to start is to have your physician send the referral by phone, fax, or text.
